Job Description
Assists in identifying and utilizing community resources for patients and their families. Liaisons with various agencies regarding social work issues and maintains all pertinent records.
Job Responsibility
- * Assists in identifying and using community resources for patients and/or their families including visits to patient’s homes.
- * Assists in establishing liaison and communications with other agencies including visits to agencies.
- * Maintains a variety of records and reports relating to social work activity
- Performs related duties, as required.

Job Qualification
- Associate of Arts/Science Degree or equivalent college coursework, in Behavioral and/or Social Sciences, required.
- Demonstrated interest in the field of Social Welfare via educational, previous employment, fieldwork, volunteer work, and/or life experience, required.

The salary range for this position is $27.55-$29.91/hour.
