Job Description
Contacts and schedules patient/client’s appointments for consultations, meetings with community agencies and/or diagnostic tests. Sets-up and maintains files and records. Collects and organizes statistical reports. The Outreach Services Associate is responsible for delivering community-based health services, including rapid testing for HIV, syphilis, and hepatitis (Hep B & C). This role involves engaging diverse populations in community settings, providing education, conducting screenings, and linking individuals to care and support services.

Job Qualification
- High School Diploma or equivalent required.
- 0-1 years of relevant experience, required.
- Previous experience in HIV / HIV prevention case management, preferred.
- Ability to communicate effectively while being sensitive to cultural diversity.
- Prior experience with the LGBTQ community, preferred
- Ability to work evenings and weekends, as needed.

The salary range for this position is $36980.00-$55690.00/year.
