Develops, organizes, integrates and oversees initiatives, special projects and programs related to inpatient care coordination services and activities across the Health System. Coordinates administrative/clinical aspects. Ensures operational effectiveness of inpatient care coordination programs, inclusive of staff, operating efficiency and meeting the goals of Northwell Health and value based care coordination.
Job Responsibilities • Leads a functional unit in multiple sites with larger portfolios by developing, communicating, and building consensus for goals/programs that support business or region Develops and articulates a short-term (6 months – 1 year) strategic vision for areas of responsibility. Directs the clinical team in the roll out and implementation of the care coordination model at the site and throughout the health system. Develops metrics, processes and reports to monitor care coordination performance inclusive of productivity, process and outcome measures by department/program and at the health system level. Develops and recommends strategies for various care coordination models related to care transitions. Assists in the development and monitoring of payroll, supply and equipment budgets. Identifies care coordination trends and opportunities for improving patient care; maintains current knowledge of and ensures compliance with all regulatory and professional standards of care. Develops and implements policies, standards and procedures that support and facilitate effective transitions of care. Chairs various committees and programs with an emphasis on improving care coordination at system level. Selects, develops, manages and evaluates direct reports; and oversees the selection, development, management and evaluation of indirect reports. Identifies and implements appropriate staff development activities in conjunction with site and system leadership. Performs related duties as required. Job Experience & Qualifications • Master’s Degree in Nursing or related field required, or equivalent combination of education and related experience. Bachelors or Masters must be in Nursing. • Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed. • 8-12 years of relevant experience and 7+ years of leadership / management
