Job Description
Develops numerous strategies for education and facilitates health curriculum for patients living with HIV/AIDS. Connects patients with resources to overcome food insecurity challenges, and other Social Determinants of Health.
Provides direct case management and care coordination services to people living with HIV who are out of care (OOC), newly identified in care (NIC), or at risk of falling out of care (RFC). Conducts comprehensive intake assessments, develops individualized care plans, and provides ongoing care coordination to support sustained engagement in HIV care and viral suppression. Work in collaboration with the multi-disciplinary team (Medical Provider, Nurse, Social Worker, and Health Education Team). Facilitates linkage to internal and external supportive services and provides health education to enrolled clients. Provides Risk-Reduction counseling, works directly with Sexual Health.
Job Responsibility
- Provides in-person/virtual group education for patients
- Develops curriculum, lectures, flyers necessary for outreach and education
- Provides Education, Health Counseling, and facilitates support group(s) in coordination with the HIV care team.
- Works closely with program staff to streamline access to internal resources and external resources for new and/or difficult-to-retain clients.
- Administers Intake/Reassessment process, and collects patient eligibility documentation
- Assists with care coordination, case conference and documentation of patient programming activities.
- Works within the HIV division as determined Operations Supervisor.
- Participates in multidisciplinary team meetings; and represents the program at external conferences, meetings and training.
- Maintains logs of activities, referrals, and outcomes. Maintains other records as required.
- Performs related duties, as required.
Job Qualification
- Bachelor’s degree required, or equivalent combination of education and related experience. Master’s degree preferred.
- 1-3 years of relevant experience, required.

The salary range for this position is $66300.00-$75012.00/year.
