Job Description
Provides drug information and assists the interprofessional care team in providing optimal pharmaceutical care for patients. Completes required learning experiences and select elective learning experiences, assigned residency projects, and educational competencies, goals and objectives as required by the individual program and as defined by the American Society of Health-System Pharmacists. Acts as a pharmacist when licensed in New York State
Job Responsibility
1. Dispenses medication.
• Interprets and enters drug orders and patient data into computer profile. Selects, prepares and dispenses drug products to fill patient prescriptions. Labels drug products according to procedure.
• Interacts with health care personnel to provide drug information.
• Screens for drug allergy and drug-drug, drug-food and drug-herb interactions.
• Conducts admission/discharge patient medication reconciliation upon request.
• Reviews patient medical record and nursing medication administration records for accuracy and completeness to maintain an accurate pharmacy patient profile.
• Manufactures, prepares and labels compounds, solutions and other sterile products as, required.
2. Participates in clinical practice.
• Provides comprehensive pharmaceutical care to improve medication usage and therapeutic outcomes. Monitors ad assesses drug therapies to insure optimal therapy regimens.
• Maintains records of activities and effectiveness of drug therapy regimens.
• Participates on multidisciplinary committees.
• Participates in the development or medication of policies related to medications, medication-use evaluations, adverse drug event prevention, monitoring and reporting.
3. Participates in medication use process
• Participates in managing and improving the medication-use process.
• Provides evidence-based, patient-centered medication therapy management with interdisciplinary teams.
• Reviews appropriateness and safety of medication orders.
• Prepares and dispenses medications following existing standards of practice, and operating policies and procedures.
• Documents significant patient care clinical interventions and resulting actions in the Pharmacy Clinical Intervention System.
• Participates in the development and maintenance of the hospital formulary.
4. Participates in education.
• Participates in the educational training and competency programs for the pharmacy staff.
• Participates in interdisciplinary (medical/nursing/pharmacy) educational programs regarding medication safety initiates.
• Utilizes medical informatics.
• Publishes period newsletter or bulletins for healthcare providers on timely medication-related matters and medication use policies.
• Responds to drug information inquiries from healthcare providers.
• Conducts educational programs about medications, medication therapy, and other medication-related matters for healthcare providers and to the community through wellness programs.
5. Performs related duties, as required.
Job Qualification
• Doctorate of Pharmacy (PharmD) from an ACPE accredited college, required.
• Eligible for licensure in New York State.

The salary range for this position is $30-$30/hour
